One Rehabilitation Service offers brain injury services at the client’s home and local community. We provide therapy in the home to optimise learning and transfer of skills. As an interdisciplinary team, we use a client-centred model and provide goal directed therapy. We understand that everyone has different circumstances and that no two people are the same. That's why we tailor rehabilitation programs around your goal and needs.
We offer a full complement of allied health professionals that work together as a team. Team members communicate daily about your progress to enable efficient and responsive changes to your rehabilitation program. The team would meet with you and your family regularly to review your progress and identify new goals. Our focus is on improving skills and developing training strategies, to increase independence and participation in everyday activities. We use evidenced based rehabilitation techniques and work alongside clients to strive towards achieving their goals and making positive changes in their lives.
A consultation service is offered, that includes single discipline or multi-discipline assessments and reports. Interprofessional assessments for a clients care needs and community care plans are also available. For clients living in regional areas, our clinicians have access to telerehab.
We can facilitate the transition from hospital to home or supported accommodation. The home environment will be assessed for home modifications and equipment prescribed as required. We also organise carers and assist in their training specific to your needs.
We can work with your personal carers or assist in the organisation of new carers. We train carers for care needs such as transfers, positioning, self care and feeding. We can also work with families and carers to set up therapy and exercise programs that can be scheduled into daily routines.
We offer a team of professionals experienced in implementing positive behaviour support plans to help with this complex area. The team works together to comprehensively assess the behaviors of concern, their functions, and causes. A plan would be developed with all involved, which would integrate the principles of positive behaviour support to address the individual needs of the client and their situation.
Clients with spasticity are assessed interprofessionally by a neurophysiotherapist and/or an occupational therapist. Evidence based treatment, including casting and Electrical Stimulation (ES) and EMG triggered Electrical Stimulation (ETS) is available post botulinum treatment.
